Item actions are applied to standard Freeway items (images, tables, form items etc.) where they will generally modify the code that the item generates.
Select an item you wish to add the action to and choose Actions/PHP feedback form from the Item menu (see figure 1.1).

Alternatively you may wish to apply the action using the apply action button in the Actions palette (figure 1.2).

The resulting dialog box will display a list of all page actions including PHP feedback form. Select this from the list and click OK.
If Graphics Preview is switched off in the View menu the item will display the name of the action attached to it to indicate the action has been successfully applied (figure 1.3).
